The slice selection gradient (G_s) should be created by a combination of y and z gradients.
G_z = G_s Sin 45^o
G_y = -G_s Cos 45^o

The phase encoding gradient (G_f) should be created by a combination of y and z gradients.
G_z = G_f Cos 45^o
G_y = G_f Sin 45^o

The frequency encoding gradient (G_f) will be equal to the x gradient.
G_x = G_f
